Many of you have asked us if we could make our liability and release forms available online for easy access when traditional paper versions are not handy. We are happy to present the following standard forms below, which you are free to reprint, photocopy and fax in lieu of the paper versions. We will be adding more forms as the need arises.  We hope this will help you with planning attendance at youth events and facilitate compliance with legal guidelines.

1.   Adult Release Form -- The standard liability and release form for adults

2.   Annual Form w/Medical -- The standard liability and release form along with medical information valid for all events in the parish for  one year

3.   Event Form w/Medical -- The standard liability and release form along with medical information for a specific event

4.   Medical Bilingual Form -- The bilingual medical form in both English and Spanish (must be accompanied by #5 or #6)

5.   Annual Bilingual Form -- The bilingual liability and release form in both English and Spanish valid for all events in the parish for one year               (must be accompanied by #4)

6.   Bilingual Release Form -- The bilingual release form in both English and Spanish for a specific event (must be accompanied by #4)

7.   Adult Sports Form -- The standard liability and release form for adults for sport events

8.   Youth Sports Form -- The standard liability and release form for sport events

9.   Trips Release Form -- The standard liability and release form for events requiring travel

10.   Employee Affidavit Form -- An Affidavit of Good Moral Character which must be signed by all paid employees who work with youth and witnessed by a notary public

11.  Volunteer Application Form -- The official Archdiocese of Miami application form for prospective volunteers which must be signed and dated

12.  Code of Conduct Form -- The Code of Conduct must be filled out and signed by all volunteers and employees who provide services for minors and/or vulnerable adults

13.  Driver Questionnaire -- The standard volunteer driver form must be filled out and signed by all volunteers and employees who transport minors and/or vulnerable adults in their vehicles

14.  World Youth Day Adult Release Form -- The designated World Youth Day 2008 liability and release form for adults

15.  World Youth Day Teen Release Form -- The designated World Youth Day 2008 liability and release form for teens
